You can also download a loan credit application at Medicard.com Just complete the application and fax to Medicard at 1-888-689-9862 and be sure to leave a copy with the CLI coordinator.
CLI also accepts payment for programs by credit card including visa, mastercard, American express and debit card. You may also pay by cash or cheque (sorry no out of country cheques accepted).
